Online Application 2024-2025

Instructions

1) Use Mozilla Firefox/Google Chrome browser to Apply Online.
2) Date of Birth of child should be between 01 Dec 2018 to 30 Nov 2019.
3) Apply by clicking the "New Application" tab.
4) Fields marked with "*" are mandatory. Do not submit the application without filling the mandatory fields. Incomplete forms will be rejected.
5) After entering the details, you may save the application. Application Form Number will be displayed.
6) Make sure to note down your Application Form Number, which will be the reference for all future correspondences.
7) After saving, you can edit the application if required.
8) You can submit the application through "Final Submission" button. Please note that once submitted, you cannot edit your application.
9) Take a printout of the submitted application form and sign the "Declaration" part.

View/Edit Application Status

After submission of Online Application, You can view/edit Application status using "View Application Status " tab or "Edit Application Status" tab by entering Application No and date of birth which you have already provided.

How to Print Application Form

After "Final Submission" of online application, You can Print Application form using "View Application Status" tab by entering application no date of birth which you have already provided.
